
Pathways Foundation
The Pathways Foundation generates vital funding for programs and services at Pathways through charitable gifts, grants, and community partnerships. These funds ensure quality care and comfort for patients and their families; including services for un/underinsured individuals, our 24/7 Dream Team, and Bereavement Support Program. The Foundation also enables Pathways to innovate to meet community needs through programs; like We Honor Veterans and Advanced Care Planning.
When you give a charitable gift to Pathways, you touch the lives of others. You probably won’t ever meet them in person, but it could be:
- A 39-year-old woman without health insurance who didn’t expect to be dying so young
- A distraught elderly man in a grief support group that your gift helped ensure
- A grieving family who just lost a child and whose bereavement volunteer was specially trained thanks to your gift
- A woman receiving chemotherapy whose symptoms could not be managed without the palliative care program supported by your donation
- A patient overwhelmed with multiple medications who received a home visit from a Pathways pharmacist

Do you know someone who needs a hug? The Pathways’ Bear Hug Program ensures that everyone who needs extra comfort is able to receive it. The program is entirely funded by Pathways community members, who are dedicated to sharing warm fuzzy teddy bear hugs with those in need. Please contact the Foundation Department at 408-730-1200 if you would like to request a bear or make a donation to the program.
