February is Pathways Home Health & Hospice Month Care with Kindness and Dignity for Over 37 Years

Sunnyvale, CA — February 13, 2014 — Pathways Home Health, Hospice & Private Duty has adopted February, the month of Valentines, love and hearts, as Pathways Month. With its heart shaped logo, Pathways is celebrating its mission of caring for patients and families with kindness and dignity for over 37 years in the San Francisco Bay Area.

Pathways Home Health, Hospice & Private Duty serves patients and families in the Santa Clara, San Mateo, San Francisco, Alameda, and parts of Contra Costa Counties from three regional offices in Sunnyvale, Oakland and South San Francisco.

Pathways Home Health, Hospice & Private Duty cared for 5,632 patients and their families in the past year. During that same period, Pathways volunteers provided 21,575 hours of comfort and care to hospice patients. The Pathways bereavement counselors held 378 group grief sessions, 4,846 individual counseling sessions and made 11,187 grief support visits.

Pathways Home Health, Hospice & Private Duty offers several comprehensive patient care programs that include home health, hospice, palliative care, private duty, and medication management. Pathways recently enhanced several home health programs such as chronic disease management, telemonitoring, and pharmacist assistance to enrich the care provided in the homes of our patients and to help them avoid rehospitalization.

Pathways Home Health, Hospice & Private Duty also benefits from gifts and donations given to them by the community. These gifts help fund services that would not be offered otherwise including, family bereavement counseling and support, integrative therapies, caregiver support, and comfort care. Two upcoming benefits for the Pathways include the Links to the Heart Golf Tournament on July 14, 2014 and its 24th Annual One from the Heart Awards Breakfast which will be held October 9, 2014.

Pathways also recruits and trains volunteers to support patients and families. These volunteers make a true different in someone’s life with the gift of time. Volunteer training is ongoing and conveniently scheduled for on Saturdays. After completing the class, volunteers can provide respite for caregivers, offer companionship for the patient, run errands and help families and patients as needed.
Grief support and bereavement remembrance events are also community services provided by Pathways. Upcoming 2014 Evening of Remembrance events will be held this year in Oakland on May 4, 2010 and another in San Mateo on May 18, 2014.
