Meet the Pathways Home Health Team

Nurses

Nurses visit to assess, do hands-on care, and teach about disease and medication management, and pain and symptom control. Specialties include IV therapy and wound and ostomy management.

Physical and Occupational Therapists, and Speech Pathologists

Based on patients’ personal goals, therapists develop coordinated rehabilitation plans that promote mobility and daily activities, often for people who have had a stroke, fracture, joint replacement, or trauma.

Social Workers

Social workers provide short-term counseling, financial and resource planning, and assist with making long-term care arrangements as needed.

Home Health Aides

Home Health Aides provide friendly assistance with personal care such as bathing, dressing, and changing the bed linens.

Pharmacist

Pathways’ Home Health pharmacist sees patients with complex conditions and many medications.  She teaches how best to take medications and reviews drug lists for possible side effects and optimal treatment.

Respiratory Therapist

Having a respiratory therapist in Home Health is unique to Pathways.  She sees patients who have lung conditions to assess how to best manage their disease.  She consults with nurses who have questions about lung patients with complex care.

Diabetic Educator

This is a nurse expert who has special training in care of diabetics.  She teaches about nutrition, exercise, blood glucose monitoring, injections, disease process and signs to watch for.  She is also updates other nursing staff to keep all of them abreast of diabetes news.

Dietitians

Dietitians consult with the team to evaluate diet and teach meal planning to reach your health goals.

Volunteers

These are trained, caring people who give their time providing companionship to patients, as well as caregiver relief, and practical assistance.
