Service Committments
Pathways strives to provide service excellence to our patients, their families and the healthcare professionals who refer to us. Fulfilling these promises is the top priority of Pathways employees. We make the following commitments:
Quality
Pathways promises to maintain our high quality care ratings. We will always provide proactive, expert pain and symptom management and treatment.
Timeliness
We will contact patients or their families within two hours of receiving a referral. With all necessary documentation we will admit the patient that day, or within 24 hours of when family or responsible parties can be present.
Communication
Calls will be answered promptly by a knowledgeable operator who will connect you to the appropriate department quickly and efficiently. From 5:00 p.m. to 8:30 a.m. calls will be answered directly by a nurse. We will give families and doctors updates based on the method and frequency requested.
Patient-centered Care
Each patient will have a care plan tailored to their individual needs. We will always put the needs and wishes of our patients first and will be available to our patients and their families 24 hours a day.
Culturally Sensitive Care
Pathways will always be sensitive to the religious and cultural beliefs and practices of our patients and families. We will do our utmost to find caregivers of similar backgrounds if desired.
Confidence
We promise to provide patients and caregivers all the instruction necessary (verbally, in written materials and by demonstration) so that they can confidently maintain optimal care at home.
Integrity
Pathways will always adhere to strict ethical standards and act in the best interest of our patients. When advised of a concern we promise to address it that same day. Our care will always be delivered with kindness and respect.
Technology
Pathways promises to continue to lead in innovative technologies that provide more accurate, speedy, and cohesive care to patients and make us more responsive to referring healthcare professionals.
