Information for Donors

Making a tribute gift in honor or in memory of someone important in your life is a very special way of recognizing those who are important in your life.

The family of those you remember or the special person you honor will be notified of your gift, without mention of the amount. Please note the person you wish to honor on your check and/or the form you send us, as well as the name and address of the person you wish to be notified of your gift.

Pathways is honored to serve your family and loved ones. When you remember Pathways as a recipient for tribute donations, you help ensure that we will be here to serve others in the years to come just as we assisted those you love. As you honor those you love, you help other patients and families in their hospice journey.

If you choose to designate Pathways Home Health & Hospice as your charity of choice in lieu of flowers, please include in the obituary or other death notice this mailing address for memorial donations:

We are also happy to provide you with donation envelopes to make available at a memorial service. As donations are received, we will send a thank you letter to the donors and send you a list of all donors. Please let us know who in your family should receive the notification of gifts. When donations total $5,000 or more, you may set up a Remembrance Fund to honor your loved one.
